Welcome to Wealth from Health™
Jersey City Medical Center-Liberty Health's innovative, Reward Program for our
staff and patients


We know that patients with chronic disease benefit from high-quality, coordinated outpatient care. This coordinated care includes patient-managed tasks, interventions and educational activities that are 'best practices'. Best practices have been shown through studies to improve health outcomes including a better quality of life, fewer complications, less hospitalizations, and lower overall healthcare costs.


How does it work?


WHO: Patients or employees of Jersey City Medical Center who wish to best manage their chronic diseases.


WHAT: A reward system for reaching self-management goals. For patients and their families, points are earned and can be 'cashed' in for gift cards or products. Being part of the Wealth from Health™ club also gets you discounts at local vendors in your community.


WHEN: From the moment you join, you will receive a photo ID card and you're in!


WHERE: Our current vendors are located in the Jersey City neighborhood but the list is expanding as more participants sign-up.

For primary care, we offer access to JCMC's Inner Circle of physicians who are located throughout Hudson County and include our partnering Federally Qualified Health Centers


WHY: We all respond best to rewards that are immediate and can be held in our hands. When prioritizing our day, taking care of our health can fall below the many tasks we need to do for ourselves and our families. So giving you an incentive to pick up your medicine, fill out a chart, or get your flu shot for example is a smart use of 'healthcare resources' and can help you live a healthier and happier life.
